Does your son or daughter need adaptive technology, hearing aids, personal assistance funding?

These are just a few examples of items that can be funded through the Kentucky Hart Supported Living Grant.   Learn more at Hart-Supported Living Program

This funding  opportunity is available  to individuals that may or may not be on the  waiting lists for Kentucky waiver programs through this competitive grant process.  Income eligibility is 300% of poverty level based on family size. However if your son or daughter is receiving SSI  or SSDI family income is waived.  

 

The Deadline to submit applications for  the Kentucky Hart Supported Living Grant is April 1,2020 DAIL-HSL-01 ApplicationHowever, to allow time for corrections to your application, it is highly recommended  you submit your application by March 1,2020.

A regional co-ordinator will review your application and make suggestions if needed.
